Hey Marcin,
I enabled debug logging on the wiki, which may provide some visibility into any ConfirmAccount issues.
I didn't see anything about debugging the ConfirmAccount extension from its documentation

But I dug through the code and saw some calls to wfDebug(), which should drop some logs into our main wiki debug log, if it encounters some odd exceptions. The only thing is that the mediawiki debug logs are *very* verbose, and it appears to stop writing after the file grows to 2.1G.
Let me know if this issue occurs again. And be sure to send me the date/time they registered, username/email address used to register, any relevant error codes, and screenshots.
